New clients: Before your first visit there will be a few things for you to do to make your therapy process go as smoothly as possible. Please print off the following forms (located on the left), thoroughly complete each document to bring to your first visit, and carefully follow the instructions as they pertain to your specific situation.

Clients with Insurance Benefits:
In-network services for Aetna, PacificSource, HealthNet, Samaritan, Value Options, and MHN members are provided with Fulfillment Family Therapy. Be sure to contact your insurance company to inquire about your in-network mental health benefits prior to your first session. Just because we are in-network does not mean your specific plan covers all services. Please bring the following information to your first session: Deductible and co-pay information, as well as Photo ID and Insurance card.Out-of-network insurance benefits: If you are using insurance benefits that are not from the companies mentioned above, then be sure to inquire about your out-of-network mental health benefits specifically. If this is the case, you will want to be aware of your plan deductibles, co-pays and reimbursment rates prior to scheduling. We accept all out-of-network insurance benefits. This includes all companies such as Blue Cross Blue Shield, United, Providence, etc. Please bring us the following information to your first visit: Deductibles, copays and reimbursment rates, as well as Photo ID and Insurance card. ** If your plan does not cover out-of-network mental heath in an affordable way, please let us know. Your therapy is important to us and we can then discuss a cost effective payment arrangement with a sliding scale/cash pay plan.
